Potential stories about changes in health care delivery are abundant ...

Many of you probably saw the recent Sunday New York Times Magazine story about home- and community-based health care, called ?What Can Mississippi Learn From Iran.? Basically, it describes a still rudimentary and not fully funded vision of creating ?health homes? using community health workers to reach the medically underserved in the Mississippi Delta, particularly those with chronic diseases. The idea is based on a similar program in Iran.

There?s a lot I liked in this article by Suzy Hansen. The author is an American living in Turkey and she?s not a health care writer. She came to the subject ? and to the Mississippi Delta ? with fresh perspective. I liked her focus on chronic disease, disparities (black/white, rich/poor, rural/urban), prevention and wellness, the need for more and better primary care, the lack of care coordination and the rationale for more integrated delivery systems with primary care at its core. She did that well. Hansen also did include some context (I would have preferred a bit more) about some of the other delivery system changes unfolding through the Affordable Care Act, changes in the delivery of health care that my posts here often urge you to report on.

So the points I raise below aren?t because I think the article was bad ? as I said, I liked it. Rather it?s because I wanted to draw out a few points that will help you as health journalists read it a little more critically and then think a little harder about what?s going on in your own states or communities.

Hansen said in an interview that she first got intrigued about the Mississippi initiative after seeing an earlier report on it in 2010 AARP Bulletin. When I went back and read the 2010 article, and then re-read the Times piece several times I wasn?t really clear on how much the ?health homes? had achieved in the two intervening years. I emailed her and she said, ?It?s all moving slowly. They have implemented the HealthConnect program, they have trained community health workers, they have designed the health worker certificate program at Jackson State, and opened clinics in some of the schools. They are trying to demonstrate various aspects of the model in order to convince people to fund them.? But the to-do list is still long.

Given all the attention right now on changing health care to achieve precisely the goals that the Mississippi group is trying to achieve (wellness not sickness, better care coordination, fewer hospitalizations, reducing disparities, etc). I was left wondering what?s stuck here ? why isn?t this program accomplishing more. (It might have to do with Mississippi and local politics and personalities ? not purely ?health.?)

But that?s something you can look at:

  • Who in your community seems to have a good idea and isn?t getting anywhere or is moving more slowly than anticipated? Why?
  • What ? or who ? are the barriers, the change-resistors?
  • Is there someone else in the same region with a similar approach that has gotten off the ground?
  • Is there a community elsewhere in the country with a very similar idea that is either up and running or about to be?
  • Why can town/city/county/health system X do this and town/city/county/health system Y can?t get past the ?talking about it? stage?

Hansen?s article did mention the shifts in health care the ACA aims to achieve. She noted, briefly, that the health law recognizes the need for all these changes contemplated by her subjects in Mississippi. She mentions the ?hot spots? that Atul Gawande, M.D., has written about, and has a quote from Richard J. Gilfillan, M.D., director of the Center for Medicare and Medicaid Innovation (CMMI) about encouraging more primary care-centered models and more preventive health. But there was still a bit of a disconnect ? that somehow this ?health home? model has to be imported from Iran. I know (from personal experience!) that editors often like ideas that sound counterintuitive and exotic: Pariah Iran can cure what ails the Mississippi Delta.

But there are scores of other programs across the country engaged in the same kind of transformation as this Iran-focused team in the Delta. Small patient-centered medical homes, school-based clinics (which is a part of what they are doing in Mississippi), community health centers that are trying to bridge social and medical needs (they vary ? there?s a fair amount of innovation going on in the best of them), growing numbers of integrated delivery systems, the ?hot spotters,? accountable care organizations, community health workers/health navigators, care transition teams and community-based geriatric teams trying to prevent newly discharged hospital patients from boomeranging right back into the hospitals, visiting nurses (not all of whom are quite as 9-to-5 and profit-driven as described in the article, although there are obviously significant problems in that industry). There also is a lot of work going on developing ways to use telemedicine and new technology to monitor frail or chronically ill people at home. There are many and they are growing all the time. Hansen?s article wasn?t designed to tell readers about all of them ? she wanted to focus on a particular community and the people trying to heal it.

?I was most interested in why this 80-year-old man who has been working in poor communities all his life turned to this concept, and what that revealed about American health care and the lives of people in Mississippi,? she emailed me. ?In the end, I felt that most average Americans are not aware of these various initiatives ? of any alternatives to American-style health care ? and that it was worth it to highlight this story as one example of thinking outside of the box.? (She also noted that, to many New York Times readers, Mississippi is almost a foreign country ? and vice versa.)

Hansen wasn?t trying to make the case that the Mississippi effort was the be-all of health reform. Which means she left plenty for the rest of us to write about.

Like the one in the Delta, the new models and initiatives aren?t all fully formed. They won?t all meet expectations, and some will probably fail. But there is a growing recognition ? and it?s spreading ? that new models are needed. (And they don?t have to come from Iran.)

Moving to these models doesn?t always require more money. It does require redirecting the money already being spent. Changing payment incentives ? and the flow of money to hospitals and specialists and acute care and emergencies instead of a more holistic community-focused chronic disease priary care-based management approach ? is not going to happen overnight. It will take years. But it?s a change that is under way ? in Boston and Philadelphia and Denver and Texas and Iowa and Camden and Alaska and the Bronx ? and in countless communities in between.

How to find them? One way is CMMI. For instance, I searched for ?Mississippi? in the Center for Medicare and Medicaid Innovation Center main page and came up with 51 hits (some of the listings seemed duplicative, and some were run by out-of-state entities, like Duke), I looked at the CMMI Innovators map - and found zero in Mississippi (most were concentrated on the two coasts and the northeast).

I looked at the Partnership for Patients page at CMMI, and came up with three pages of Mississippi projects (just because they are there doesn?t mean they are all big, ambitious or successful, just that they?ve joined the initiative). The point isn?t that the Iran-model isn?t a good or interesting or smart one - it is interesting. It?s just that if you, as health journalists, look around you will find that there are a lot of other people who aren?t just yelling and screaming about health reform. They are also trying to make it happen.

One last thought ? not all the pilots and projects out there use as much home-based care as the Mississippi project contemplates for the non-elderly. If you come across a project using community health workers/health navigators let me know. I know of a few too?? maybe we can highlight them in a future post.

There are those that believe that because many of these illnesses have been around forever, that maybe we just have to stay with them. Until now that may well have been the case. Luckily, however, technological innovation has now caught up to this section of the health and fitness health care program as well. It can now carry condition prevention and health and fitness stages of skills not only up to, but even surpass, the innovative level of efficiency that most of the rest of the health and fitness health care program presently enjoys.

Usually any way of successful entry into the health and fitness health care field is introduced directly through this sort of profession itself, whether it includes the introduction of new equipment, new tools, new systems, or new concepts. Furthermore, those improvements are usually carefully developed around assisting the present healthcare program. The program being suggested here is developed to carry more balance into the overall health and fitness health care program with far more emphasis on the person?s and public?s best interest. This would be accomplished by presenting into the health and fitness health care program an efficient precautionary and health and fitness program, generally by means of an inclusion to the present healthcare program.

The suggested program includes the inclusion of a very sophisticated nationwide healthcare details program, very unique for a number of reasons. First, not only is it developed to more completely provide the needs of the patient and the community, but it?s also a program that would be mandated into everyday living and controlled by the community through a community commission.

Lets read a couple of paragraphs from the publication that I believe you will find very interesting, one includes the bio-chemical make-up of the body system (primarily what CLS is based on) and the second talks about significant success recently made in scientific clinical technological innovation and what this success could mean to us.

(AP) ? Penn State's season opener this Saturday will be the first since 1949 not featuring someone with the last name Paterno on the coaching staff. They're out of the picture, like the campus statue of the family patriarch; erased like his coaching records.

But the family will be in Beaver Stadium in spirit, if not watching from a skybox ? a sign that through the turmoil of the past few months, life goes on for the clan that was synonymous with Penn State for decades and remains firmly rooted in a supportive community.

Scandal tarnished the legacy of the late Joe Paterno, the Hall of Famer who coached the Nittany Lions until his firing in November days after former assistant Jerry Sandusky was arrested on child sex abuse charges. Son and former longtime assistant coach Jay Paterno no longer tutors the quarterbacks.

Jay Paterno, one of Joe and Sue Paterno's five children, has been traveling and writing columns at a time when he's usually used to getting ready for the season opener.

"It's such a change in your lifestyle. Credit to Jay. Jay has kept himself busy. He's smart enough, savvy enough," said Tim Sweeney, a businessman and former player who is head of the Football Letterman's Club. Sweeney, who said he can relate as a former high school coach himself, briefly saw Jay Paterno at a charity golf outing in early August.

"Still, their hearts will be at Beaver Stadium on Saturday," said Sweeney, referring to Jay Paterno and the other veteran assistants who left after the arrival of Bill O'Brien, Paterno's replacement as head coach.

The family, as part of Paterno's employment agreement, received use of a Beaver Stadium suite for 25 years. Family members intend to attend the season opener against Ohio University on Saturday, but only to show their support for the new regime, family spokesman Dan McGinn said.

"The family doesn't want to do anything to distract from the game this weekend," he said.

Affixed to the front screen door of the Paternos' modest ranch home is a copy of a blue and white sign also on display in the windows of most downtown businesses, some homes and even cars. The message on the sign reads: "Proud to Support Penn State Football."

Members of the Paterno family either did not return calls or would not comment for this article. But the community of State College, often known as Happy Valley, appears to remain protective of the closest thing it has to a royal family.

Neighbors said that while Sue Paterno might venture out a little less than before the scandal hit, grandchildren, other relatives and friends still stop by the house regularly. Well-wishers, strangers among them, drop off Penn State paraphernalia or other messages of support in the front yard or doorstep.

Most in the broader community who talked to The Associated Press said what happened was tragic and they're sympathetic to the sex-abuse victims, but they perceived a rush to judgment about Joe Paterno's role in the scandal.

Sue Paterno continues to volunteer with Special Olympics, a charity she has long championed. A new Catholic student center across the street from campus bearing her name appears near completion.

"We are so grateful to her and the entire Paterno family for their dedication, prayers, and support," the Roman Catholic Diocese of Altoona-Johnstown said in a statement. "Because of Mrs. Paterno and so many others, Catholic students at Penn State will soon have a place to call their very own."

Sue Paterno stayed mostly out of sight at a summer pep rally for the team, save for a brief appearance at a side door of Penn State's football building. Unseen by most of the dozens of fans at the early morning rally, she was there and gone ? quickly.

Lawyer and former Penn State defensive back Adam Taliaferro, now a university trustee, grew close to the Paternos after suffering a serious spinal cord injury in a game against Ohio State in 2000. He said he spoke to Sue inside the football building while fans rallied outside and told the AP she was "one of the strongest women I know."

"All of us together will help her get through it," Taliaferro said. "She will always remain a huge part of the university."

Paterno started with Penn State as an assistant coach in 1950, becoming head coach in 1966.

Former FBI Director Louis Freeh's university-commissioned report in July said Paterno and three other school officials concealed allegations against Sandusky. Ten days later, the school took down the bronzed statue of Paterno outside Beaver Stadium.

Then, the NCAA announced it had accepted the findings and levied unprecedented penalties on the program including a four-year bowl ban, significant scholarship cuts and a $60 million fine. The NCAA vacated every Penn State win from 1998-2011, and Paterno was stripped of 111 career victories ? meaning he no longer holds the record for most coaching wins in major college football.

Paterno, who was diagnosed with lung cancer days after his firing, was not interviewed by Freeh's team, and the family said the NCAA had not contacted the family or the family's attorney. He died in January.

The family has said university leadership and the NCAA accepted Freeh's conclusions in a rush to judgment, without due process and a questioning of the findings. They vowed after Freeh released his findings that they would conduct their own investigation, which is ongoing, McGinn said.

"They're not looking for sympathy," McGinn said when asked how the Paternos have reacted to criticism. "The family is very stoic about this. The intent is just that the complete truth comes out."

We haven't heard a lot about TiVo's Stream transcoder since it was officially unveiled in May, but today emails went out to winners of a Facebook contest informing them they can pre-order one for $129 (plus shipping) and receive it on September 5th. In case you've forgotten what its capabilities are, the notice shown above from Zatz Not Funny also includes a description of its capabilities which include streaming shows to up to three different mobile devices (iPhone, iPad or iPod Touch) in the home at once, streaming a show to your mobile as it's recorded, or wirelessly transferring shows to the device to view them on the move, TiVoToGo-style. Those interested will of course need a Premiere series DVR that's hard-wired to a router, but with that it's just a one time fee to easily cut the tether on TV content. Interestingly, a few TiVo Community posters report being able to preorder from the listed number without being in the contest, so if you're already intending to jump just pick up the phone and give it a shot.

Survivors: strong gas odor before Venezuela blast

PUNTO FIJO, Venezuela (AP) ? Venezuelans who live next to the country's biggest oil refinery said they smelled a strong odor of sulfur hours before a gas leak ignited in an explosion that killed at least 39 people and injured more than 80.

The Amuay refinery was still burning on Sunday more than a day after the blast, sending up a thick column of black smoke.

Residents in a neighborhood next to the refinery said that starting about 7 p.m. on Friday they noticed the unusually strong odor. Government officials say the blast occurred about 1:15 a.m. on Saturday when the gas leak created a cloud that ignited.

The cause of the disaster is under investigation.

Gabriela Nunez, a housewife who lives near the refinery, said she noticed the gas odor on Friday night and then hours later came the shock wave.

"All the windows shattered, the iron doors opened, the wooden doors broke," Nunez said. She returned to her home on Saturday night to gather belongings, saying she was worried about looters who had stolen goods from nearby stores hours after the explosion.

"That forced us to come back, even though we're afraid, to save what can be saved and secure our houses," Nunez said.

A total of 209 homes and 11 businesses were damaged in the explosion, and a National Guard post next to the refinery was destroyed, Vice President Elias Jaua said late Saturday. He said 18 of the victims were National Guard soldiers.

President Hugo Chavez on Saturday declared three days of mourning in the country.

Amuay is among the world's largest refineries and is part of the Paraguana Refinery Complex, which also includes the adjacent Cardon refinery. Together, the refineries process about 900,000 barrels of crude per day and 200,000 barrels of gasoline.

Oil Minister Rafael Ramirez said the country has enough fuel in storage, "10 days of inventories," to keep the Venezuelan market fully supplied with gasoline and diesel.

He said fires were still burning in two fuel storage tanks on Sunday but that other "process areas" of the refinery have been otherwise unaffected. Once the flames are completely extinguished, Ramirez said, "we have the ability to restart our refinery in two days."

Riots as Kenyan backer of Somali Islamists killed

Deadly riots broke out in Kenya's main port of Mombasa on Monday after the assassination of a radical cleric linked to Somalia's Al-Qaeda-allied Shebab militants.

At least one person was hacked to death as thousands of angry protestors took to the streets after Aboud Rogo Mohammed -- who was on US and UN sanction lists for allegedly supporting the Shebab -- was shot dead.

"A car behind us aimed at my husband, they shot him on the right side," said his widow Haniya Said, screaming in grief after the killing by unknown attackers.

"One person has been killed, he was slashed to death during the protests," said regional police chief Aggrey Adoli.

Cars were set on fire and two churches were looted in the city -- Kenya's main port and a key tourist hub -- according to an AFP reporter.

"There is chaos in town now, and our officers are on the ground dispersing the rioters to maintain peace," added Adoli. "They are demonstrating against the killing of Aboud Rogo, who was shot by unknown people."

Witnesses said that Mohammed's car was riddled with bullets, and a photograph released by his supporters showed his bloody corpse slumped behind the wheel of a car.

"He died as we rushed him to hospital. Why have they killed my dear husband?" his widow added, before she and her children were taken to the hospital.

Mohammed was placed on a US sanctions list in July for "engaging in acts that directly or indirectly threaten the peace, security or stability of Somalia", specifically for recruiting and fundraising for the hardline Shebab.

The United Nations Security Council placed a travel ban and asset freeze on the cleric in July, saying he had provided "financial, material, logistical or technical support to Al-Shebab".

He was the "main ideological leader" of Kenya's Al Hijra group, also known as the Muslim Youth Center (MYC), the UN said. The group is viewed as a close ally of the Shebab in Kenya.

Mohammed "used the extremist group as a pathway for radicalisation and recruitment of principally Swahili-speaking Africans for carrying out violent militant activity in Somalia", the UN said.

MYC leader Sheikh Ahmad Iman Ali, in a message posted on Twitter, said: "We are on the right track when our leaders get shahadah (martyrdom)."

"He will remain in our hearts forever," the MYC added, while another message offered the grim warning that the "kuffar (infidels) will pay" for his death.

"The whole city is on fire, there are looters in the streets, cars have been damaged, some have been burnt," said Francis Auma, from the local organisation Muslims for Human Rights.

The local Muslim Human Rights Forum condemned Mohammed's assassination, claiming it "mirrors" the recent killings or disappearance of others "on the country's terrorism watch list".

Mohammed "repeatedly called for the violent rejection of the Somali peace process", the US Treasury said, noting he had often advocated the use of violence against both the UN and the African Union force battling the Shebab in Somalia.

He "urged his audiences to travel to Somalia to join Al-Shebab's fight against the Kenyan government", the Treasury added.

Kenyan police arrested the preacher in January, seizing firearms, ammunition and detonators, but later released him on bail.

He was previously acquitted of the 2002 bombing of an Israeli-owned hotel near Mombasa which killed 15 people -- 12 Kenyans and three Israelis -- as well as three suicide bombers.

The cleric is also alleged to have introduced Fazul Abdullah Mohammed -- the late head of Al-Qaeda's east Africa cell, shot dead last year in Somalia's war-torn capital Mogadishu -- to at least one of the men who helped him carry out the twin US embassy bombings in 1998.

The bombings in Nairobi and Dar es Salaam killed 224 people.

Mohammed, born on Kenya's Lamu island, was aged between 43 and 52, according to different aliases.

Here Are Apple's Plans To Make TV A - Business Insider

Apple's invasion of television may not be the revolution everyone dreams of, but that doesn't mean it's not going to introduce some revolutionary new features.

Jessica Vascellero and Sam Schechner at the Wall Street Journal have new details on Apple's vision for the television market.

Sadly for many people, Apple's still going to work with cable companies, which means consumers won't be free of $100+ cable bills. But for those $100+ bills, Apple is going to make the user experience entirely more delightful if it can get the cable companies, as well as content companies, on board with its plan.

Here's what Apple is pitching, according to Vascellero and Schechner:

  • Apple wants to make what amounts to a souped-up version of Apple TV that can function as a cable set-top box.
  • You would be able to start any show at any time using a DVR that stores TV shows on the Internet.
  • The TV interface would look like an iPhone or iPad, with icons that users navigate. (Perhaps it looks like the current Apple TV interface with a few more icons?)
  • The new interface is supposed to be better and easier to use than the current cable box interfaces. (A pretty low hurdle to clear.)
  • There would be a way for people to share on Twitter or Facebook what they're watching on TV.
  • Users would be able to get the entire season of a show on demand, as opposed to a few episodes the way cable companies offer now.

It all sounds like a much better TV experience, but people shouldn't get their hopes up just yet. Apple not only has to get cable companies on board with the plan, but the content providers, as well.

The cable companies only have rights to do so much with the content they broadcast. Apple would have to convince companies like Viacom, the NFL, MLB, Time Warner and others to sign up.

These companies are not dummies. They're not going to just walk into a negotiation with Apple and say, "Sure guys, take whatever you like." They going to ask for some money. And that money will then be passed on to users.

So, the questions that remain are how much are they looking for, how much is Apple willing to pay, and how much will it cost users?

Until those questions are answer, the talk of Apple building a better way for people to watch TV is just talk.

But, boy, is it exciting talk.

I felt like I was learning a whole new language. I became quite frustrated and confused with the huge volume of information that was there, especially when so much of it was written in medical jargon. I have a PhD in biochemistry, yet I still found the information confusing!

Then there were those websites selling all sorts of herbs and potions, and promising miraculous cures. The cynic in me said ?I guess if people are desperate they?ll try anything?. I wondered if I would get to the stage of trying these alternative therapies, and if so, how I would choose between them.

She had had personal experience with melanoma (with a family member) and at the time, she explored many options regarding treatment and did a lot of research. That?s what the production of?Cirque du Soleil?s Dralion?received on Wednesday, 8/15/12, its opening night in Raleigh?s PNC Arena. And it was well deserved. If you do not have plans for this weekend, you should definitely take your family to check out this delightful and critically-acclaimed show.

Dralion mixes components of traditional Chinese acrobatics with modern Western circus. Its name is a fusion of the ?dragon,? which symbolizes the east, and the ?lion,? which represents the west. And then there?s an additional layer ? the four elements of nature and the conflicts and balances between them. It helps that the elements are color-coded: air is blue, water is green, fire is red and earth is ochre. In the world of Dralion cultures blend, Man and Nature are one, and balance is achieved.

As with other Cirque du Soleil shows, Dralion is a feast for your senses. First and foremost, the performers were phenomenal. They were athletic, graceful and reminded me of some of the gymnasts in last week?s Olympics ? always in control, perfectly synchronized when required. Most the acts are performed with no safety nets or cables, so the timing of their routines has to be perfect. The acrobatics are unique and show off the performers? incredible talents.

The costumes were stunningly beautiful, eye popping in fact. According to Dralion?s publicist, Julie Desmarais, more than 16,000 feet of fabric were used to create them. The set is amazing as well, especially for a traveling show. It looks like a futuristic Chinese temple and includes a 60? x 26? wall that spans the length of the stage. It becomes a central part of the show as the artists climb and suspend themselves from it, performing amazing feats, some of which involve trampolines, where performers are literally bouncing off walls. Ms. Desmarais said it takes 18 semi-trailer trucks to transport the production from city to city.

The music in Dralion is an integral part of the show. The music is performed live at every show by six musicians who play a wide variety of instruments, including the violin, cello, percussions, drums, electric and classical guitars and keyboard. The score is heavy on Eastern rhythms but also pulls from classic rock and new age stylings and complements each act.

Dralion has all of the elements one would expect from a circus: jugglers, trampolines, balancing acts, flying acrobats and clowns, don?t forget the clowns. The Dralion clowns interact with audience throughout the show and keep the audience laughing, young and old alike. Much of their routines are slapstick and vaudeville-like.

I have seen two other Cirque du Soleil shows in Las Vegas (Myst?re and ?O?) and this one lived up to my expectations. I think it will live up to yours as well.

French mayor denies "Brangelina" to wed at weekend

PARIS (Reuters) - A French mayor on Tuesday denied a report in celebrity magazine Hello that quoted him saying Angelina Jolie and Brad Pitt, who own a chateau nearby, were due to get married this weekend.

The A-list Hollywood couple own the 17th-century Chateau Miraval in southeastern France, and celebrity publications have been rife with rumors in recent weeks that they were planning to wed soon.

"I don't know who they are," said Michael Latz, mayor of the village of Correns where the couple dubbed "Brangelina" have their chateau, when asked if he had spoken to Hello.

He said he was not aware of any wedding, and added: "It's nonsense. I've had 50 phone calls and I am telling everyone to get lost because this is tiresome and irritates me.

"I am going to pursue them (Hello) for misleading statements."

Hello magazine reported on its website on Tuesday that the mayor had said Jolie and Pitt would marry this weekend, although it did not quote him directly.

Jolie, 37, and Pitt, 48, announced their engagement in April but have not publicly set a wedding date. The couple have been together since 2005 and are raising six children.

(ARA) ? Buyers are still clamoring for real estate deals in this turbulent market. Foreclosures and short sales offer some of the best bargains, but also have a higher risk level. Still, more than four in five adults think buying a foreclosure or short sale can be good deals, according to a recent American Society of Home Inspectors (ASHI) survey.

Some analysts say the rebound has begun and home prices may rise by the end of 2012. This means now may be buyers? last chance to take advantage of affordable properties and low interest rates. If you want to score a bargain before the housing market recovers, you?ll need to follow a few rules to invest with certainty.

Make a wise investment by adhering to these five rules while shopping distressed properties:

Rule 1: Position yourself for success

Before starting your search, get preapproved for a mortgage so when a good deal presents itself, you?re positioned to submit a bid right away to be the first offer on the bank?s desk. Work with an experienced real estate agent who can help guide you through the daunting sea of foreclosures and short sales. Bidding can be complicated and time-consuming, especially when working with a home sale needing bank approval. A good agent will know how to navigate through the paperwork and red tape.

Rule 2: Do your research

A real estate agent can help you with research, but it?s wise to do some on your own. Are there any undisclosed liens on the property? Is the seller behind on his property taxes? What permit records does the city have on file? This information will be critical during decision-making. Work with your agent to ensure the contract requires any delinquent taxes, liens or assessments will be paid prior to you taking ownership of the property.

Rule 3: Always get a home inspection

Eighty-four percent of adults surveyed by ASHI said they would be more likely to purchase a distressed property after a home inspection has determined its condition.

A home inspection gives you the confidence to move forward with your purchase because you?ll have as much knowledge as possible about the condition of the property. An inspector will visually examine the condition of the home?s roof, attic and insulation, foundation, basement and structural components, as well as interior plumbing and electrical systems. Be sure to find an ASHI-Certified Inspector (ACI) to ensure your inspector is experienced, as many states have minimal licensing requirements. Rule 4: Budget for repairs

When looking at buying a foreclosure or short sale, remember price is only one aspect to consider. A home will almost always require some type of repair. After receiving your inspection report, you can estimate costs associated with necessary repairs, maintenance or energy-efficient improvements.

Rule 5: Assess the neighborhood

Location should be a top consideration when purchasing real estate, and in a tough housing market, it?s even more important. A home has limited worth if it?s located in a less desirable neighborhood. High foreclosure rates can turn a once-desirable neighborhood into one many might likely avoid. These locations are likely to see a slower recovery than more populated or favorable areas less affected by the economy. Make location as important as price when making a purchase decision.

(AP) ? The Syrian prime minister who defected said Tuesday that Bashar Assad's regime was near collapse and urged other leaders to tip the scales and join the rebel side. The U.N. said an estimated 2.5 million Syrians have been injured, displaced or face problems securing food or other necessities, a sharp rise from about 1 million three months ago.

It was the first public statement by Riad Hijab since he left his post and fled to Jordan with his family last week. Hijab is the highest-ranking political figure to defect from Assad's regime.

"The regime is on the verge of collapse morally and economically in addition to cracks in the military," Hijab told a news conference in the Jordanian capital, Amman.

Hijab is a Sunni Muslim from the eastern province of Deir el-Zour where rebels claimed to have shot down a regime MiG-23 warplane on Monday. Hijab, who was not part of Assad's inner circle, said the trip to Jordan lasted three days during which he was protected by rebels of the Free Syrian Army.

He said he felt "pain in his soul" over the regime's shelling and other attacks on rebel strongholds as the government stepped up its military offensive. Activists say more than 20,000 people been killed in the struggle since March 2011.

"I was powerless to stop the injustice," Hijab said, speaking in front of the rebel flag. He called on "honorable leaders" in Syria to defect as well.

"Syria is full of honorable officials and military leaders who are waiting for the chance to join the revolution," he said, adding that Assad's forces only control 30 percent of Syria.

"I urge the army to follow the example of Egypt's and Tunisia's armies ? take the side of the people," he added.

Hijab said he was now backing the rebels, but gave no clue on his plans. There had been speculation that he would travel to the Gulf nation of Qatar, which is one of the rebels' main supporters.

The Britain-based Syrian Observatory for Human Rights said at least 45 people, including 29 civilians, were killed in Tuesday throughout Syria while the Local Coordination Committees, another activist group, said 50 people lost their lives. The Observatory said six members of the same family, including two children, were killed when troops shelled the eastern village of Shumaita.

In Geneva, the U.N. said that its humanitarian chief has begun talks in Syria on a mission to boost international aid inside the war-battered country. Valerie Amos was to meet with Syria's foreign ministry and the Red Crescent, which has been the pipeline for humanitarian supplies to Syrians caught in the civil war.

Jens Laerke, a spokesman for the U.N. Office for the Coordination of Humanitarian Affairs in Geneva, said Amos is on a three-day visit to the region.

She said the U.N. now estimates that more than 2 million Syrians have been injured, displaced or facing problems securing food or other necessities. Also, more than 200,000 refugees have fled to neighboring countries including Turkey, Lebanon and Jordan.

"We have a situation where there is an ongoing humanitarian crisis," Amos said in Damascus after meeting National Reconciliation Minister Ali Haidar. "Three months ago, we thought that there were about a million people who were in need of assistance. We're now having to revise that number to about 2.5 million," she added.

"I think it's important that all parties to the conflict recognize that it is ordinary men and women who are caught up in the middle of this and that it is important that the fighting stops," she added.

A spokesman for outgoing U.N. peace envoy Kofi Annan said that Syria authorities have backed former Algerian Foreign Minister Lakhdar Brahimi as his successor. The spokesman, Ahmed Fawzi, said the next step was for Brahimi, a longtime U.N. official, to formally accept the post and resume efforts for a diplomatic solution to Syria's crisis.

In Lebanon, the militant Hezbollah group denied Tuesday that a man captured in the Syrian capital of Damascus and shown in a video released by the rebels was one of its members.

The video is the latest incident to reflect rising sectarian divisions in Syria's vicious civil war, which has seen an increase in abductions of Shiite Muslims who many rebel fighters perceive as supporting Assad. The regime is dominated by members of Assad's Alawite minority sect, an offshoot of Shiite Islam. Sunnis, who are the majority in Syria, make up the backbone of the opposition.

The video purporting to show the captured Lebanese man followed another highly circulated rebel video Monday, showing the downing of the Syrian MiG and armed men later holding the captured pilot who ejected. Syria acknowledged a pilot had bailed out of a disabled plane but blamed the crash in Deir el-Zour on a technical malfunction.

In the video with the Lebanese captive, a man identifies himself as Hassane Salim al-Mikdad, and says he was one of 1,500 Hezbollah fighters sent to Syria on Aug. 3. The video was said to have been released by rebels and aired by Arab satellite TV Al-Arabiya on Tuesday.

"Most of those who entered were snipers," said the captive, whose face showed bruises as three masked gunmen stood behind him. A man, who could not be seen, was asking the hostage questions.

The captive then says that Hezbollah leader Sheik Hassan Nasrallah gathered the men before they headed to Syria and told them that they should go to "support the Shiite regime and the Shiite army against Sunni gangs." The authenticity of the video could not be independently confirmed.

Hezbollah, a Shiite Muslim group, is a key backer of Assad's regime. The Syrian opposition has repeatedly accused it of sending fighters to Syria, which Hezbollah denies.

Hezbollah issued a statement early Tuesday saying it "categorically denies that Mr. Hassane Salim al-Mikdad is one of its members."

There have been several attacks and abductions in Syria of Shiites from Lebanon, Iran and Iraq over the past months that were blamed on Syrian rebels. In May, Syrian rebels captured 11 Lebanese Shiites shortly after they crossed from Turkey on their way to Lebanon.

Earlier this month, 48 Iranians were captured by Syrian rebels near Damascus. Rebels claim the Iranians include members of Tehran's Revolutionary Guard and were on a "reconnaissance mission" in the Syrian capital. Iran insists the men were on a religious pilgrimage.

The Lebanese are apparently held to try to pressure the government in Beirut to show greater support for the Syrian rebels ? which is unlikely because of Hezbollah's strong influence and backing of Assad.

Also, many Iraqi Shiites, streaming back to their homeland in the past month to escape the conflict in Syria, reported a rash of attacks against their community, apparently by Sunni rebel gunmen. In July, 23 Iraqi Shiites were killed in Syria, some of them beheaded, according to the Washington-based Shiite Rights Watch. In one gruesome case, the U.N. said an Iraqi family of seven was killed at gunpoint in their Damascus apartment.

The motives for the attacks on Iraqis are unclear. They may be revenge against any Iraqi because the Shiite-led Iraqi government is seen as siding with Assad.

In other violence across Syria on Tuesday, activists reported clashes and shelling in the northern city of Aleppo, southern province of Daraa, suburbs of Damascus and the northwestern region of Idlib.

State-run Syrian TV said gunmen shot and killed Maamoun al-Zoebi, deputy director of the health ministry in the southern province of Daraa, as he was leaving work. The station said four gunmen shot him dead then took his car and drove away.

On Monday, gunmen abducted the correspondent of Iran's Arabic-language TV al-Alam in the central city of Homs Ahmad Sattouf, his wife told The Associated Press. She said he was taken from his office and his whereabouts are unknown.

Sattouf becomes the third journalist to be abducted in Syria over the past three days. Three other journalists were killed over the weekend in Damascus and its suburbs.

State-run news agency SANA said one of its reporters was wounded Monday while covering clashes in Aleppo.

It?s really easy to go to a party intending to stick to your healthy intentions but forgetting about eating healthy when you see your mom?s famous blueberry pie. And of course you need whip cream on top and ice cream later! Summer is a time for cookouts, parties, car trips and days at the beach and all of these fun adventures usually come stocked with greasy food and high calorie snacks.

Here are a few easy steps to take to stay healthy at a cookout or party without depriving yourself:

1. Don?t ?save? up your meals for the day.

I know it can be tempting to go hungry all day so you can eat more at a party but honestly this plan never works. By starving yourself all day you?ll be slowing down your metabolism because your body will think you are literally starving.

If you know you have a party at night eat healthy and filling foods leading up to it. I always recommend having a little snack before getting ready to ensure that you do not beeline to the dessert table and can actually enjoy yourself and your company. Some suggestions: an apple with peanut butter, carrot sticks, cup of berries, or veggies with hummus.

2. Pick your indulgence.

Is there something you have really been looking forward to eating and enjoying? Well by all means, go for it! Stick to one serving size of that particular dish and enjoy it fully. Food is meant to be enjoyed not rushed through. When you slow down and actually enjoy your favorite desserts and snacks you?ll not only fully appreciate them but you?re body will have time to realize that it?s full and stop you from eating too much.

3. Fill up on the healthy choices?

After your indulgence move on to some of the healthier options to fully fill you up ? like grilled veggies, salad, watermelon and other types of fruit. I like to have what I?ve really wanted first so I?m not mindlessly eating the fruits and veggies in anticipation for my indulgence. Instead, I enjoy the fruits and veggies even more after I?ve satisfied my craving.

4. Watch the drinks & ?picky? food

Lemonade and iced tea can have a lot of added sugar. Instead stick to water with lemon or orange slices for a refreshing summer drink that will keep you hydrated without all the sugar. Another thing that is always at a cookout is lots and lots of snacks. You know, the food where you can just pick at it while you talk and not realize that you?ve had 30 chips. It?s so easy to mindlessly eat but see if you can step away from the bowl and fully enjoy a friend or family members conversation.

If you really want chips take a small handful on a plate and walk away from the buffet table. Don?t stand next to it and let yourself inhale an entire bag.

5. Enjoy the event not just the food

Realize food is meant to nourish you. Be present when you are eating and chew slowly. Most importantly, enjoy your company and have fun!

Sears spinning off some stores into separate co.

(AP) ? Sears is preparing to spin off its Hometown and Outlet stores as well as some hardware stores into a separate publicly traded company.

The news sent the company's stock up $2.58, or 5 percent, to $54 in premarket trading Monday.

Sears had signaled that it would split the companies back in February. There are a total of 1,238 Hometown, Outlet and hardware stores as of April 28, according to Sears, which is trying to turn around its business and spruce up its image. It has already closed five Hometown stores, eight hardware stores and one Outlet store.

Sears, which also owns Kmart, has looked at spinoffs and real estate sales to restore profitability and boost shareholder confidence. Aside from the separation of the Hometown, Outlet and some hardware stores, the company announced in May that it would spin off a stake in its Canada division.

Sears did not disclose Monday in the filing with the Securities and Exchange Commission how much money it expects to raise with the spin-off of the Hometown, Outlet and hardware stores. It previously said that it expected the deal to raise between $400 million and $500 million.

Sears Chairman Edward Lampert's ESL Investments Inc. will own a majority stake in the business being spun off.

For fiscal 2011, the combined Sears Hometown, hardware and Outlet businesses had net income of $33.1 million on sales of about $2.34 billion. That compares with fiscal 2010 net income of $49.8 million on sales of approximately $2.35 billion.

Sears Holdings Corp., based in Hoffman Estates, Ill., is set to report its second-quarter earnings Thursday.

The new company will be listed on the Nasdaq under the "SHOS" ticker symbol.
